A Clinical Study of Reopertaion for Intraabdominal Abscess



Abstract
This report included the clinical analysis of 32 patients with postoperative intraabdominal abscesses about its incidence, symptoms, signs, etiologic distribution, hospitalized days, duration of drainage, diagnostic procedures, laboratory
findings,
operative procedures, complications and mortality who were at the Department of Surgery, Seoul Adventist Hospital during the 5 years and 3 months from May, 1986 to August, 1991.
1) Of the 32 patients in the series, the sex of male of Female was 1.3 : 1, and the mean age was 35.4 years.
2) The most common age group was third decade(28.1%) and the range was 7 to 72 years.
3) The common previous operation of the intraabdominal abscesses were appendectomy and drainage for perforated appendicitis or periappendical abscess(34.3%), and operation of hepatobiliary system(78.1%).
4) The common symptoms and signs were fever and chillness(87.5%), abdominal pain(81.3%), localized tenderness(78.1%).
5) The leukocytosis was 87.5%.
6) The common location of abscess were interloop abscess(34.3%), ad subphrenic abscess (21.9%).
7) The common duration between the previous operation and symptoms onset were within 1 month(59.3%), and 1 to 2 months(15.6%).
8) The common Hospitalized days were 11 to 20 days(50%) and 21 to 30 days(25%).
9) Positive culture was obtained in 25 of 32 cases(78.1%), and E-coli was the most common microorganism(44%).
10) The common methods of treatment were explo-laprotomy and drainage(71.9%), and non operative percutaneous drainage(12.5%).
11) The complication rate was 34.3%, and wound infection was the most common complication(18.8%).
12) The mortality of 32 cases was 2(6.3%).
The causes of death were sepsis and pulmonary complication.
